Well Health Care was founded in 2018 with a mission to bring high-quality and enriched living to the lives of people with disabilities across Melbourne. Our team is made up of passionate and purposeful care workers dedicated to delivering a higher quality of life. We provide disability and NDIS support, aged care support, private care, and group activities.

Knowing we have been able to make an impact on the lives of so many thus far gives us the drive to continue to deliver on our mission now and into the future. Everyone deserves to live an equally fulfilling and enriched life and we are dedicated to helping you and your loved ones achieve this in many forms.

The experienced team of support workers at Well Health Care have curated a number of key services to enhance your quality of life. Our services all aim to ensure everyone has equal opportunity to live an independent, fulfilling, and sustainable life that is not defined or withheld due to disability of any kind.
We provide services across Melbourne, aiming to impact the lives of every client and family we engage with. Our services are available for people of all ages, ranging from young children through to the older members of the community.
Well Health Care is a registered NDIS provider with trusted and experienced support workers across Melbourne. Disability support allows people living with a disability to have the support and guidance needed to achieve their desired goals and live the life they want.
Disability support is there to encourage independent living, while having the security of knowing you have someone advocating for you along the way. The Well Health Care team are passionate about empowering and encouraging all clients to fulfill their dreams.
Support workers aspire to help people with disabilities unlock their fullest potential including learning new skills and capabilities and stepping outside of their comfort zone.
We understand you want to provide the very best support for your loved ones as they begin to need that extra support and care. Our experienced support workers are here to alleviate any concerns for you and your loved one. Aged care encompasses assisting the elderly members of our community to enjoy the life they are living without having to put strain or pressure on those closest to them.
There is no single way to care for someone, at Well Health Care we provide bespoke programs and facilities to cater to each client’s individual needs. We work with you to identify the kind of support and services you feel would be suited to you and your family.
While everything around you may be changing, it is nice to keep your home comforts near. At Well Health Care, we offer tailored private care inside the comfort of your own home. The private care service offers you a top-level of support delivered by our passionate and experienced support workers.
Private in-home care offers the flexibility to stop or start care at any given time, which gives you that extra freedom and peace of mind. Having care delivered to you in your home means you are given the one-on-one personalised experience with the undivided attention of your support worker. This service can be self-funded or through government funded packages pending your eligibility.

Staying connected and engaged in the activity is important to enhance your quality of life, we offer group activities including:

Well Health Care appreciate the importance of stepping outside of your comfort zone and taking on new experiences, participating in our range of group activities gives you or your loved ones the opportunity to connect with other people who may share similar interest or simply a chance to make a new friend.

Assess your current

The Well Health Care Team will sit down with you to discuss yours or your loved one’s individual needs, goals, and aspirations to ensure we find the most appropriate service. We understand this can be confronting and ensure we have a team of passionate, kind and understanding care workers who will make this experience as smooth as possible.

Discuss the support services
we have available

We will sit down together to discuss the many services and sub-services available to you. This will help us identify which program would best suit you and your needs. We always strive to deliver the best care possible and ensure this step helps us achieve this.

Create and implement a plan
to optimise your experience

Now that we have assessed your current circumstances and identified which service would best suit you, we will create a plan to get you started as soon as possible. Please be assured we can make changes to your service at any given time should you decide it isn’t right for you.

HOW WE CAN SUPPORT YOU

On top of providing tailored care to each of our clients, we are also able to support you throughout the entire process. We pride ourselves on upholding a high-level of organisation and communication between you and your loved ones, this ensures we can stay on top of things and that you are getting the care you deserve.

A support worker is a professional who is responsible for assisting those living with disabilities both physically and mentally to live a fulfilling life whilst encouraging independent living. 

It is recommended you engage in a support worker when you or your loved one requires assistance performing day-to-day tasks. 

Yes, we have NDIS, Aged Care, Private Care and Home Care Packages. Get in touch to discuss further.  

The NDIS is available to people who have a disability caused by a permanent impairment. We recommend visiting https://www.ndis.gov.au/applying-access-ndis/am-i-eligible to see if your circumstances are applicable. 

To apply for NDIS, you would need to submit an NDIS access request form. This is available along with further information on the NDIS website or by clicking the link here https://www.ndis.gov.au/applying-access-ndis

 Connecting with your support worker is important, if you aren’t connecting with your support worker, we can work with you to allocate someone more suitable.  

WHC have several group activities including Arts and Crafts, Music, Games, Community events, Shopping and Sport. 

A referral form can be found on our referral page, please ensure you have all key information available.

Yes, we have a team of registered nurses with more than 10 years of community nursing experience who able to assist and coordinate with medical professionals to provide a top-level of care to you. This service will be conducted in the comfort and safety of your own home.
